Working with Children Screening Checks*	Background Check Consent Form   Prohibited Employment Declaration print both documents and fax all pages back to 8765 9049 with copies of supporting identification documents as follows:  100 Point Identity Check

*Oz Homestay require all host families to undertake the Working with Children Checks prior to hosting any students.  The Consent to Screen and Prohibited Declaration forms need to be completed and signed by all members of the household 18 years and over. Please print off enough copies for this procedure. Please ensure you have provided all information requested including "other names" i.e. middle, former, maiden names as omission of these will delay processing of your application. As we place students with families continuously during the year, it means we are constantly losing available families  – so we are always on the lookout for new families.  If you know of friends, neighbours, relatives who could be interested in the program, please give them our contact details. 

We are currently looking for families within 30 minutes travel time to CBD and in the St George Area.
